What did the Joker mean by his speech about everything "according to plan," in the Dark Knight?

I'm talking about the scene when the Joker is in the hospital and convinces Harvey to kill the ones responsible for Rachel's death. He talks about "everyone loses their minds," everything "according to plan." What did he mean by that?
